Why we built a rehab system inside a working rehab

Reach + Within blog card: "Why we built a rehab system inside a working rehab"

I'm Vincent Conlon. I run a rehab in Cape Town. I've owned rehabs in South Africa and in Europe, and I've been in recovery for about ten and a half years. This is the story of why we ended up building our own software, and why we're now offering it to other clinics.

The problem we kept paying for

Every rehab I've run had the same shape of problem. Admissions kept a list of enquiries in one place, often a spreadsheet or a WhatsApp group. Nurses kept medication charts and a scheduled-drug register on paper or in a separate tool. Counsellors wrote notes in another system again. Accounts kept invoices and balances somewhere else.

Each system did its own job reasonably well. None of them talked to each other. So the same details were typed three or four times, and the picture of a person in treatment was split across systems that different people could see.

We tried general practice software. It was built for consulting rooms, not for a 24-hour ward with medication rounds, withdrawal scales and family calls. We looked at specialist rehab software from overseas. Much of it was built around US insurance billing and didn't fit South African clinics.

Building it on the ward

So at our rehab we built Reach + Within ourselves, with the people who'd use it.

Our nursing team shaped the eMAR: sign and countersign, a scheduled-drug register kept alongside it, e-scripts that arrive in one inbox from approved senders, and ward and patient-own stock with counts. They also shaped the nursing admission, with vitals, weight and an automatic BMI, and the CIWA-Ar and COWS withdrawal scales with their scoring keys.

Our counsellors shaped the clinical record: biopsychosocial assessments, treatment plans, individual and group notes, and outcome scales (PHQ-9, GAD-7 and AUDIT) that clients complete on an iPad or by secure link and that graph over time.

Our admissions and accounts team shaped Reach: an enquiry pipeline with an owner for each enquiry, a waiting list, quotes, admissions, invoices, statements and aged balances.

When something didn't work, we heard about it the next morning, from someone we work with every day. That's the biggest difference between software built inside a clinic and software built for one.

What Reach and Within do

Reach is the front office and accounts side. A website enquiry drops straight into the pipeline. When someone is ready, one click onboards them and creates their file in Within, with the details already filled in.

Within is the clinical record: assessments, treatment plans, session and group notes, eMAR and nursing, scales, iPad check-ins, incident logs, secure family report links, discharge and aftercare.

Within Assist drafts notes. With the client's consent, a session can be recorded or a recording imported and transcribed. Assist drafts from the transcript and the client's history, and the clinician reviews, edits and signs. Assist doesn't diagnose and never signs the record.

An independent company, with your own private workspace

This matters to every rehab owner we speak to, so I'll be direct. Reach + Within is an independent company. Each clinic's data is kept in its own private workspace that no other clinic, including the rehab where it was built, can ever see. Within each workspace, roles decide who sees what: clinicians see clinical notes, while PA and accounts users can book and invoice but can't open clinical notes. Client data is hosted in the EU (Netherlands), and there's an audit trail of who viewed or changed what.

What's not there yet

We'd rather be honest about the gaps. Medical-aid claims are on our roadmap but aren't live yet, so South African clinics should keep their billing bureau or claims tool alongside Reach for now. If something you need isn't there, ask us. We'll tell you straight whether it's built, planned or not on the list.

Try it

Therapists, group practices and rehabs can ask for a 30-day free trial through the form at reachwithin.app. You start with demo clients, so you can try everything safely before any real records go in.

If you run a clinic and your systems make something harder than it should be, I'd like to hear about it.
