Treatment centres and rehabs in South Africa run a harder workflow than a weekly counselling diary. Admissions, clinical notes, nursing, family contact and discharge have to agree about the same person. This page describes Reach + Within for residential and outpatient programmes. The system was built inside a working rehab running across two registered facilities, where it holds up to special audit reviews by the relevant authorities. That is the line we use. We do not add a brand story beyond it.

Admissions and the clinical file

An enquiry needs an owner, a referrer and a next step. When the person is admitted, the file should already hold the details the admissions team captured. The waiting-list habits in running a list fairly apply in spirit: tell people the truth about the wait, review risk rather than leaving it in a silent queue, and do not treat software as an emergency service. Local emergency numbers and your own clinical protocol still do that work.

Care, families and review

Clinicians write assessments, plans and session or group notes. Nursing records, including eMAR where the programme needs it, are part of the quote conversation rather than a hidden extra we pretend is irrelevant. Families receive an approved report through a secure family report link, not the therapy note and not the medicines record. Consent decides what leaves the building.

Assist will draft a session note only after consent is on the file. In early admission, consent has to be real. Read recording therapy sessions before you make recording routine on a unit. The clinician signs. The software does not.

The audit trail shows who viewed or changed what, for a review that should not become an argument. Sign-in with a second factor is coming soon. Client data is hosted in the EU (Netherlands). Outcome scales are coming soon. Medical aid claims are coming soon. A treatment centre that needs scheme billing today should not hear a softer sentence than that. Say it in the quote meeting.

What the quote includes

HPCSA record keeping still applies to the practitioners on the team who are registered there. Retention, as Booklet 9 states it, is summarised in how long to keep records. A rehab that is also a registered facility may have additional duties. Ask your compliance lead. We will not invent them.
