AI session notes the clinician still signs

With consent, Assist drafts the note from the recording. You edit it and you sign it. Without consent, you write the note yourself.

AI session notes are a draft of the clinical record, produced from a recording or a transcript, which the therapist then edits and signs. They are not a diagnosis. They are not a signed note until a clinician makes them one. This page is for UK therapists and psychologists who are considering that workflow under UK GDPR. The price for the practice software that includes Assist is £60 a month per clinician. Admin users are free. Outcome scales are coming soon and are not part of the claim on this page.

What Assist does

With the client’s consent, you record in the room, record an online session from the browser, or import a recording you already made, including a voice memo. A transcript is stored on the note. You can ask Assist, in your own words, for a summary, for bullet points, or for a note in the format you use. It reads the transcript and the existing record. It drafts. You decide what is saved. You sign.

That order is the product. If a tool signs for you, or writes into the record before you have read it, it is a different product and we would not describe it as Assist. The checklist you can pin up is recording therapy sessions: consent. Formats for the draft are in SOAP notes and DAP notes.

Consent and UK GDPR

A recording of therapy is special category data. You need a lawful basis and a special category condition, explained in language the client understands. The GDPR guide walks through the controller role, the privacy notice, and hosting. It does not choose your Article 9 condition for you.

Say, before you record, what is captured, that a transcript and a draft are produced, who processes the audio, and that you will edit and sign. Say that they can refuse and still be seen. Store the answer on the file first. Stop if they ask you to stop.

Client data is hosted in the EU (Netherlands). The ICO’s adequacy regulations cover the EEA, so a UK practice can transfer personal data to the Netherlands on that basis. Adequacy is not a certification of the software or of your practice. Transcription is done by a named sub-processor. Ask us for the list and the processing agreement. We do not claim Sage, Xero, or any other accounts package as a live integration, and we do not claim two-way calendar sync.

Where a standalone scribe is a better fit

Heidi, Upheal, Mentalyc and others sell AI notes as their main product, sometimes with a free tier. If you already have a record and a diary you like, and you only want a scribe beside them, a standalone tool can be the better buy. Heidi’s public pricing page did not show figures in the HTML we read on 4 October 2026. A Heidi article stated a Clinician plan at USD $110 a user a month. Choose Assist inside Reach + Within when you want the draft on the same client as the enquiry, the booking and the invoice, and when the PA who sends the invoice cannot open the recording. That is clinical notes software connected to practice management, not a scribe tab in a browser you then copy from.

What you still do yourself

You remain responsible for accuracy. Transcripts mis-hear names and can flatten ambivalence into certainty. Read the draft. Delete what you do not mean. Sign what you do. If you would not say the sentence to the client, do not leave it in the note because the draft sounded fluent.

Roles decide who can open a recording or a note, and the audit trail shows who did. Lock the screen when you leave the room. None of those replace the consent line.

Does the client have to be recorded?

No. Recording is optional. Consent is stored on the client file before any recording, and you can always write the note yourself.

Is the draft the clinical record?

Not until the clinician reviews, edits and signs it. Assist does not diagnose and it never signs.
